What auditors check
Consultation is two-way dialogue before a decision. Participation is workers actually contributing to hazard identification, risk assessment and incident investigation, not merely receiving information.
A noticeboard, an email, or a policy poster is communication (clause 7.4) — evidence of consultation and participation looks different.
What it means for the auditor
Interview workers directly, away from their manager where possible, and test whether they recognize being consulted, not just informed.
This is the clause most likely to expose a gap between a well-written OH&S manual and what actually happens on the floor.

Hazard identification, risk assessment and legal requirements
What auditors check
Clause 6.1.2.1 requires hazard identification to be ongoing and proactive — before change, after incidents, and for routine and non-routine activities alike.
Clause 6.1.3 requires a process to identify and keep current with legal and other OH&S requirements, then evaluate compliance against them.
What it means for the auditor
Trace whether a recent incident, near-miss, or change actually fed back into the hazard register — a register that never changes is a red flag.
Test the legal register against a live regulatory requirement, rather than accepting that a register exists.

What auditors check
Competence (7.2) must reflect the hazards and risks people are actually exposed to in their role, not only a generic induction record.
Awareness (7.3) covers workers' own OH&S risks, their contribution to effectiveness, and the consequences of deviating from procedures.
What it means for the auditor
Sample people at the point of work, rather than reading a training matrix alone.
Ask what happens if a procedure is not followed — a confident, specific answer is stronger evidence than a signed induction form.

Hierarchy of controls, management of change and contractors
What auditors check
Clause 8.1.2 requires controls to be selected using the hierarchy of controls — elimination and substitution first, PPE last — not PPE as the default answer.
Clause 8.1.3 requires a documented process for managing change, including new equipment, processes and personnel changes that affect OH&S. Clause 8.1.4 covers procurement and contractors, including their OH&S performance while working on site.
What it means for the auditor
Follow one recent change, or one contractor, from induction through to on-site control and look for the evidence at each step.
Where PPE is the only control offered for a hazard, ask what was considered and ruled out higher up the hierarchy.

Monitoring, internal audit and management review
What auditors check
Clause 9.1.2 requires evaluation of compliance with legal and other requirements, and clause 9.2 requires internal audits with defined objectives, not just scope and criteria.
Management review (9.3) must cover OH&S performance, incident trends, and the status of actions from previous reviews.
What it means for the auditor
Trace an internal audit finding through to a management review input and out the other side to a documented decision.
Test whether monitoring data actually drives a decision, rather than sitting in a spreadsheet nobody reviews.

Incident investigation, nonconformity and corrective action
What auditors check
Clause 10.2 requires the organization to determine the cause, or causes, of an incident or nonconformity — not just the immediate corrective action taken.
Corrective action must be evaluated for the need for action to eliminate the cause, so that the same incident does not recur elsewhere on site.
What it means for the auditor
For any incident report, look specifically for root-cause analysis, not only "what was done afterward".

Evidence · Worker participationThe HSE manager shows you the OH&S policy and a completed hazard register. When you ask three workers on the shop floor whether they were consulted on the latest risk assessment for the packing line, all three say they first saw it pinned on the noticeboard.
Which requirement of ISO 45001:2018 is this evidence most relevant to?
Consultation and participation are not the same as communication. Clause 5.4 requires two-way dialogue before a decision, and workers actually contributing to the risk assessment. Posting a completed document on a noticeboard is communication (7.4), which happens after the fact.
Evidence · Hazard identificationThree weeks ago, a forklift near-missed a pedestrian at a warehouse intersection. You find the incident report and a completed corrective action to add mirrors and floor markings. However, the hazard identification and risk assessment register for that area still shows the same risk rating recorded 18 months ago, with no update after the near-miss.
Which nonconformity statement would stand up at the closing meeting?
Requirement, evidence, nature of the nonconformity. A defensible finding states all three and nothing more. The near-miss was investigated and a control was added — the actual gap is that nobody fed the new information back into the hazard register, which is precisely what 6.1.2.1 requires on an ongoing basis.
Evidence · Incident investigationYou review an incident report for a minor hand laceration. It records first aid given, the injured worker's name, and "corrective action: reminded operator to wear gloves." There is no record of why gloves were not being worn, whether the task's risk assessment addressed hand protection, or whether the same task is performed elsewhere on site.
What should the auditor do next?
One thin report is a lead, not yet a finding. Clause 10.2 requires the organization to determine the cause, or causes, of an incident — not just record the immediate action. Every incident is in scope, however minor. The auditor verifies with further sampling before concluding. The course trains exactly that judgment.

CQI IRCA exam structure, pass mark and procedure
The CQI IRCA exam for ISO 45001 Lead Auditor training has 40 questions worth 80 marks across five domains. To pass, you need at least 50% overall (40 of 80). In addition, you need the minimum mark in every domain. You sit this open-book exam online within 30 days of the course, and it lasts 1 hour 45 minutes.

If you pass, you receive a digitally verifiable Certificate of Achievement via Accredible within 48 hours of the result. How is the CQI IRCA exam structured and what is the pass mark?

You can resit for a AED 125 / USD 35 fee within one year, without re-attending the course. If you also fail the second attempt, CQI IRCA requires you to re-attend the course (free at 3FOLD within one year); the levy plus exam fee for that attempt is AED 310 / USD 85.
What certificate will I receive from the ISO 45001 Lead Auditor course?
Learners who pass the continuous assessment and the CQI IRCA examination receive a Certificate of Achievement, issued digitally within 48 hours of the result; those who attend but do not pass receive a Certificate of Attendance. Only the Certificate of Achievement qualifies you to apply for CQI IRCA auditor membership.
Does the ISO 45001 Lead Auditor course make me a certified lead auditor?
Not by itself. It satisfies the training requirement for initial certification as an IRCA OHSMS auditor, provided you apply within five years of completing it. CQI IRCA also requires audit experience and assesses your application before awarding the auditor grade.

Yes. A first-party audit is an internal audit an organization performs on itself; a second-party audit is performed on a supplier by, or on behalf of, a customer; a third-party audit is performed by an independent certification body to certify the OHSMS against ISO 45001.
